Our New Patient packets contain information that is necessary for our staff to properly care for you. When you come in for your first visit, we require that these packets are completed so that we may help expedite the registration process.
You can obtain these packets by printing them from the links below, requesting them to be mailed to your home, or you may arrive 30 minutes prior to your scheduled appointment time and complete them in our office.
There are 3 distinct packets available and each is required for your visit.
Part 1 – New Patient Information – This information is for you to keep, it contains detailed driving instructions and patient policies.
Part 2 – New Patient Consent Forms – Each form in this packet contains important legal information and documentation for you to approve.
Part 3 – New Patient Questionnaire – This packet contains questions specific to why you are visiting us and how we can help with your medical care.
